Senior Principal Technical Advisor for Universal Health Coverage

Job Description

The Rwanda Health System Strengthening (RHSS) project is a five-year USAID-funded activity implemented by a consortium of local and international organizations led by MSH . Working with the Government of Rwanda (GOR)’s Ministry of Health (MoH) and key stakeholders, the RHSS Project aims to strengthen and expand the performance of the Rwandan health system at the national, decentralized, and community levels. The Project’s focus areas include leadership and management, governance, health financing, private-sector engagement, quality improvement and accreditation work, health information systems, research and knowledge management. The RHSS Project contributes to Rwanda’s multi-sectoral Vision 2020 by assisting in efforts to implement the Third (and fourth) Health Sector Strategic Plan (HSSP III, IV) and building a strong foundation that will allow the entire health system, from community health workers (CHWs) to central level decision makers in the relevant ministries, to make universal and equitable access to health care a reality for all, promoting a healthy, happy and productive population.
The UHC definition captured above embodies health system strengthening elements—equity in the access to health services, quality of health services, and protection from financial risk. The Senior Principal Technical Advisor for Universal Health Coverage (UHC) will lead the design and implementation of activities under the RHSS Project’s third intermediate result (IR.3) covering health financing (HF), quality improvement (QI), and private sector engagement (PSE). S/he will be expected to work closely with the Ministry of Health, the Rwanda Biomedical Center (RBC), and the Rwanda Social Security Board (RSSB) on a wide range of HF, QI and PSE-related issues.
Reporting to the Project Director and being part of the project’s
senior management team, the UHC Advisor will work in a fast-paced, results-driven environment on a team that is responding to high priority government strategies and requests. MSH is therefore seeking a highly motivated individual with relevant technical and managerial expertise, analytical skills, and an ability to think strategically about the role of management, health economics, and financing for health systems strengthening and to build capacity of project staff, teams and units within the MoH, RSSB and other relevant stakeholders at all levels of the health sector.
As team leader for the IR.3 program area, the Advisor will oversee IR.3 team members as well as local and international consultants, review work progress toward results overcoming challenges, and contribute to overall project work planning and reporting.
Technical Leadership (80%)
The Senior Principal Technical Advisor for UHC leads the design and implementation of RHSS program under intermediate result three (IR.3) covering health financing (HF), quality improvement (QI), and private sector engagement (PSE). In this capacity the UHC leader has the following responsibilities.

• Health Insurance (35%):
o Lead RHSS support to functional analysis and adaptive strategies and reforms aimed at improving governance within the health insurance environment in Rwanda.
o Advise RSSB management on and contribute to institutional capacity building leading to improved and effective management of CBHI.
o Assist RSSB on generating and interpreting evidence to make key economic, managerial, and policy decisions to ensure financial sustainability of health insurance schemes.
o Support institutionalization of CBHI financial modeling, costing and pricing reviews, and updates to enable the projection of revenues and expenditures while ensuring equity and quality of care.
o Help the RSSB design and implementation of long term health financing strategic plans ensuring financial sustainability of the health insurance schemes and fostering further improvement of provider quality and efficiency.
o Develop cost control strategies for health insurance through designing a competitive, cost-effective and equitable benefit package and purchasing services from providers.
o Perform other related assignments related to health insurance as tasked by RSSB management.

• Quality Improvement supporting the establishment of a National Accreditation Body (30%)
o Work with the MoH Clinical Services Directorate to develop/update quality standards, policies and strategies, contribute to their effective dissemination, and support capacity building at all levels in their use.
o Work with the MoH and all stakeholders to develop the policy and legal frameworks for the establishment of an independent national Accreditation Body, and support the development of a plan to bring this to fruition
o Prepare/update a feasibility study and business plan for the creation of the Body.
o Provide managerial and technical capacity building support to the board and staff of the independent Accreditation Body.
o Advocate for sustainable financing of the Body using domestic resources (e.g. from the insurers and the providers).

• Economic, Health Financing Analysis and Income Generation (15%):
o Advise and strengthen capacity of the MoH Planning, Health Financing and Information Systems team, and broader Ministry of Health, to improve planning and evidence-based budgeting at the central and decentralized levels through institutionalized health resource tracking and economic and health financing analysis.
o Develop processes to improve budget formulation, monitor financial and program performance, and generate other routine evidence on efficiency in the health sector.
o Conduct cost, gap, impact, and other quantitative analyses that assess opportunities to improve efficiency, effectiveness, and sustainability in health financing.
o Strengthen sustainability of the Performance Based Financing (PBF) system, link PBF and accreditation at health facility level, update the indicator package for PBF at different levels
o Develop and promote innovative Community Health Worker cooperative practices for income generation, and health facility income generation strategies to drive self-sufficiency.
o Work with MoH/MoF in improving finance management capacity at all levels.
o Promote private sector engagement in the health system through Public Private Partnerships (PPPs) and other mechanisms

Project Management and Coordination (20%):
o Lead and manage the IR.3 team at the RHSS Project, including performance planning and review, supervision, and mentoring
o Participate actively in country senior leadership team meetings to discuss strategies, progress against work plan activities, challenges,
opportunities, and other project implementation matters.
o Mobilize Short Term Technical Assistance in specific technical areas when needs arise, draft scopes of work, recruit and supervise consultants, and ensure timely performance and quality results.
o Ensure that IR.3 components in the budgeted workplan are well-designed and implemented in an accurate, timely and cost efficient manner.
o Contribute to quarterly and annual RHSSA reports and other routine reporting requirements.
o Draft policy and technical briefs on relevant technical areas to maintain a high level of policy dialogue and advocacy, and ensure visibility of results and achievements to MSH, donors and stakeholders.
Education
Required
• A postgraduate degree in health economics, business, public health finance or a field of study closely related to the assigned responsibilities

Preferred
• PhD (or equivalent) in a field of study closely related to the assigned responsibilities

Experience
• Eight years of experience in designing, implementing, supporting and evaluating health services quality improvement, health economics and financing policies and programs, with particular expertise in community or national health insurance.
• Significant experience delivering technical assistance towards capacity building and skills transfer to counterparts in resource-limited settings in low or in middle-income countries.

Knowledge and Skills
• Proven leadership and collaboration with other technical advisors, experts, and host country staff from the public, private and NGO sectors.
• Demonstrated capacity to develop and implement work plans, monitor and evaluate activities, and draft reports.
• Exceptional interpersonal, teamwork and partnering skills
• Solid understanding of the Rwanda health sector and health resource environment
• Fluency in English required and in French desirable
